Job Description:
A company is looking for a Care Coordinator - CCM/RPM -
Remote.
Key Responsibilities
Manage a monthly allocation of patients, ensuring consistent care
and compliance with service standards
Conduct monthly telephonic interactions with patients to assess
health status and promote treatment plan adherence
Document patient interactions accurately and maintain HIPAA
compliance in handling patient information
Required Qualifications
Clinical credentials such as CMA, RMA, LPN, or equivalent
Minimum of 1-year experience in a clinical or patient care
setting
Strong critical thinking and organizational skills
Ability to navigate multiple computer systems, including EMR
platforms
Dependable, self-directed, and capable of managing time
effectively
